Registration Step-By-Step Instructions

Step One - Registration Page

• Make sure you are using a secure network and your internet connection is operating properly. 
• On this form, you are asked to enter all relevant Player & Parent information to register your child for the Spring 2024 Season and you will be required that you agree to the FOD Parent Authorization before proceeding to make payment. Please note, all requested fields in bold MUST be filled out to ensure proper submission of this form.
• PLEASE make sure you enter the correct date of birth. Many errors have been made in the past due to incorrect date of birth entry. Errors will only delay our draft process and may result in having a player wait listed if they are initially placed in the wrong division and no additional spaces are available. 
• Our system uses PayPal. All financial information is protected via PayPal's privacy. PayPal is a TRUSTe licensee. Learn more about TRUSTe licensee certification and what it means to you.
• The custom FOD On-Line Registration form allows for single player registration only. If you have more than one player participating in FOD’s Spring 2024 Season, please register each player individually.
• If your registration contains any errors, they will be presented in a pop-up window listing any errors, so you can continue to update the page without losing any previously entered information. Please note, ALL requested fields in bold MUST be filled out to ensure proper submission of this form.
• When hitting the "Submit Information" button, only click ONCE. Multiple clicks may result in a browser error and you will have to begin a new registration.
• If you experience any problems with FOD’s online registration form, please review this section for any trouble shooting and make sure you have provided all the information required. If you have completed all the information and are still experiencing problems submitting your form, please contact Please contact FOD's Registration contact, Jaclyn Conatser at Jaclyn.Waterman@gmail.com or the FOD President, Gene Kolisnyk at uke123@aol.com immediately. 
•  If your registration contains no errors, you will be presented with a Confirmation Page (proceed to Step Two below).

Step  Two – Confirmation Page

• The Confirmation Page provides the following:
  • The information you entered on the Registration Form.
  • Gives you an opportunity to review information on the Registration Form and make any changes you might like before completing your player’s registration.
• Click the "Pay With PayPal" Button" button once you’ve confirmed all Player & Parent information is correct. You will now be presented with the Secure FOD PayPal Payment Page (proceed to Step Three below).
• FOR 2024, THERE IS NO REGISTRATION FEE, SO PAYPAL WILL NOT BE NECESSARY; however, if you wish to make a donation to FOD it would be appreciated. 

Step  Three – Secure FOD PayPal Payment Page 

• The Secure FOD PayPal Payment Page allows you to make payments using one of the most secure online payment methods available. Most importantly, this page allows you to complete your transaction without exposing your payment information to FOD or any FOD Board or Staff. Your information is kept strictly confidential and secure with PayPal.

  • If you already have your own PayPal account, you are offered the opportunity to login to your PayPal account to complete your online transaction.
  • If you do not have a PayPal account, you will be offered an opportunity to create a new PayPal account during the online transaction process.
  • You DO NOT need a PayPal account to make your payment online. You can always just enter your credit card information in the PayPal secure Website payment page to complete your transaction.
• The bottom line about completing your FOD Online Payment is:
  • You are making payment via PayPal
  • You are never exposing your banking information or a credit card number to FOD Board or Staff 
  • You will receive a receipt from PayPal upon completing your online payment and you will have direct access to PayPal to resolve any questions you might have about your on-line payment.
• Upon successfully completing your online payment you will be returned to the FOD Payment Confirmation Page (proceed to Step Four below).

Step  Four – FOD Payment Confirmation Page 

• The FOD Payment Confirmation Page provides you with final confirmation that your online payment has been completed with PayPal.
• You should print this page and attach it to you PayPal receipt for record keeping of your completed Registration to FOD’s Spring Season along with any corresponding information regarding your on-line Payment via PayPal. 
• If you have any questions or would like to provide any feedback regarding FOD’s online registration form, please contact our Webmaster. 

IMPORTANT NOTE: If you are having trouble submitting the online registration form, please review this page for any trouble shooting and make sure you have provided all the information required. If you have completed all the information and are still experiencing problems submitting your form, Please contact FOD's Registration contact, Jaclyn Conatser at Jaclyn.Waterman@gmail.com or the FOD President, Gene Kolisnyk at uke123@aol.com.  If you do not receive payment confirmation or are experiencing difficulties with the PayPal payment process, please contact PayPal directly.
